JOB TITLE:                      LGBT ELDERLY CARE SUPPORT AND RESEARCHER

LOCATION:                      Office location is based in Stockton but area of work will be required to take place in Middlesbrough.

JOB PURPOSE:               To highlight key areas of support needed for older LGB/T people in Middlesbrough.

                                        To ensure that the needs of the older LGB/T community in Middlesbrough are being met and raised awareness in the wider community promotes community cohesion.

                                        To ensure access to appropriate services for older LGB/T people across Middlesbrough.

HOURS:                           16 per week @ £11 per hour.

RESPONSIBLE TO:         Sarah Lewis - Manager

FIXED TERM FOR 6 MONTHS

Funded by Ageing Better Middlesbrough

SPECIFIC DUTIES:

  1. Ensure a range of opportunities are made available for older LGB/T people living in Middlesbrough to access appropriate services to meet their needs.

 

  1. Engage with socially isolated older LGBT people and provide support to reduce feelings of social isolation and improve well-being.

 

  1. Promote the work of Hart Gables throughout the wider community.

 

  1. Consult with older LGB/T people and relevant professionals about barriers within this community.

 

  1. Plan and deliver a key event that focuses on raising awareness surrounding key issues.

JOB TITLE:                      Health and Wellbeing Youth worker

 

LOCATION:                     Based in offices in Stockton, but will be required to work across the Hartlepool and Stockton Borough.  

 

 

JOB PURPOSE:              To ensure young LGB/T people with mental health issues are supported through one to one and group work.

 

                                          

HOURS                             28 per week @ £13.50 PER HOUR

RESPONSIBLE TO           Sarah Lewis – Manager

FIX TERM FOR 12 MONTHS

This post is “Funded by Catalyst Stockton On Tees/NHS Hartlepool and Stockton-On-Tees Clinical Commissioning Group”

 

SPECIFIC DUTIES:

 

  1. Support young LGBT people who suffer with mental health issues – improving emotional health and wellbeing/reducing feelings of isolation/improving levels of self-esteem and reducing engagement in risky behaviours.

 

  1. Improve referral systems within the locality (Hartlepool and Stockton Borough Council, MIND, CAHMS, local schools and colleges)

 

  1. Improve health screenings amongst the young LGBT community to encourage early diagnosis.
  2. Develop a number of recommendations that will inform future working practices with this target audience.
  3. Promote the work of Hart Gables throughout the wider community
  4. Support key events throughout the year

 

 

  1. Due to the size of the organisation the post holder will be required to become fully involved in the work of Hart Gables and will be expected to undertake other duties as necessary

                                           people across Teesside.

                                           To ensure the needs of the LGBT community are

                                           met and raised awareness in the wider community

                                           promotes community cohesion  

                                          

HOURS                             20 per week @ £11 PER HOUR

FIX TERM FOR 12 MONTHS

SPECIFIC DUTIES:

 

  1. Ensure a range of opportunities are made available for LGBT living in Teesside to access appropriate services to meet their needs

 

  1. Engage socially isolated LGBT people and provide support to reduce feelings of social isolation

 

  1. Increase numbers of LGBT people suffering same sex domestic abuse to access services, report incidents and seek support
  2. Increase numbers of LGBT reporting hate crime incidents and support Hart Gables third party reporting service
  3. Improve health and wellbeing of LGBT service users – improving confidence and life chances
  4. Promote the work of Hart Gables throughout the wider community
  5. Support key events throughout the year

 

  1. Support the Manager to deliver on the outcomes of the Tees Valley LGBT Strategy
